Medical Claims Tips

Since we are part of the Blue Cross Blue Shield of Illinois PPO network, by using a BCBS PPO physician and hospital you would save money two ways. Initially, you would have less out of pocket expense since the charge to you is generally lower if you use a PPO provider. Since the Fund also saves money because of your reduced charges, the money saved could be used by the Fund to provide future Plan improvements.

If you do not have a Blue Cross Blue Shield of Illinois PPO physician check out the BCBS link (this link is also on the Health & Welfare Main Page) to find a physician in your area. After selecting a physician you just need to present your BCBS ID card at the physician’s office. Do not give them a claim form to complete and do not pay any money at the doctor’s office. You will be billed by the doctor’s office for your share of the bill after the Fund considers the claim for payment.

Although you do not need to have the doctor complete a claim form if you use a PPO physician, you will need to submit a claimant’s statement if the claim is due to an accident or if you have other insurance. You could request this form directly from our website or call us at 630 668-7260. Remember, also that if a family member has other insurance, we will need copies of the other carriers Explanation of Benefit forms.

After the Fund Office considers the claim for payment, you will receive an Explanation of Benefits form directly from the Welfare Fund. The actual payment to the doctor or hospital will be sent from BCBS.
