If you find that you need surgery or have to go to the hospital, you, your doctor or the hospital should contact the Fund’s pre-certification provider, Med-Care Management at 1 800 375-9173. This information is shown on the back of your BCBS ID card. If Med-Care Management were notified of the admission or surgery, the $100.00 basic deductible would be waived. This means that if your bills do not exceed $8,000.00 for the hospital or surgeon or any related medical bills during the surgery or hospital stay, you may not have to pay any money out of your own pocket.

If you do not use a BCBS physician or hospital, you would need do have a claim form completed by you and the doctor and be responsible for payment of the bill whenever the physician’s office or hospital makes demand for payment. In addition, both you and the Fund would not receive reduced charges.
