DENTAL CLAIMS The dental benefits are paid by the Fund and we do participate in the Blue Cross Blue Shield PPO network. When you have a dental claim, contact the Fund Office on our website benefits@smw265.org or call the Fund Office at 630 668-7260 for a claim form. VISION CLAIMS You have the choice of using an eyecare specialist of you own choosing or selecting an eyecare specialist out of the EyeMed Managed Care network. If you choose your own eyecare specialist, you will need a claim form that could be requested by contacting the Fund Office on our website benefits@smw265.org or calling the Fund Office at 630 668-7260 for a claim form. Benefits would be paid directly by the Fund Office for charges incurred up to the maximum allowable under the Plan. You would be responsible for any balance due. If you use a provider under the EyeMed Managed Care network, you could receive an exam, and glasses without any charge to you. In addition, no claim form would be needed.
